Summary
If your organization is interested in applying for the Accountable Health Communities grant, your initial Letter of Intent is due to CMS on February 8th. This grant application requires both active participation from the state Medicaid agency, Department of Medical Assistance Services (DMAS), and a contract between the applicant and DMAS. DMAS is supportive of this opportunity and given the tight time-frame has asked VCHI to gather initial information from interested applicants across Virginia. As a first step, DMAS requests when submitting your Letter of Intent to CMS on February 8th, please forward a complete copy to VCHI at AHC@vahealthinnovation.org. Shortly after February 8th, VCHI staff will be reaching out to each applicant team to gather additional information on behalf of DMAS. Once DMAS reviews the information, they will reach out to you for further conversations. This will occur the week of February 15th.
The Letter of Intent requires a DMAS point of contact. Please include Ashley Hazelton, Ashley.hazelton@dmas.virginig.gov, as the point of contact for DMAS.
